Anxiety, depression, self-doubt, and many other obstacles can make reaching your goals seem impossible sometimes. The good news is that there is always hope, and tomorrow can hold something better for you. David understands that deciding to see a counselor can be very difficult. Looking through the the phone book, websites, and directories of counselors can be confusing.

David Hage, MSW, LCSW, ACSW, C-ASWCM provides generalist practice counseling services as a Licensed Clinical Social Worker in the state of Pennsylvania and specializes in working with individual adults, older adults, and families dealing with depression, anxiety, adjustment to a disability, dementia, relationship challenges, and more. My person centered perspective acknowledges you as the expert in your own life story. Using a strengths-based approach and empathy, I help clients learn new skills, change patterns, and move forward. I have 10+ years of experience working with adults – older adults in acute care (hospital), subacute (skilled nursing facility), and outpatient care settings.

My office located in the Back Mountain (Dallas, PA) is clean, comfortable, and discrete. Working from the assumption that positive change is possible, I help those I collaborate with to better define what they hope to achieve and a realistic path to get there. Costs & Good Faith Estimate

Payment options: I currently do not accept commercial insurances and only accept private self-payment. I accept cash, check, or credit card payments due at the time of service.

Cost: My usual hourly rate is $80/hour. I believe therapy should be affordable and accesible; therefore, a reduced sliding fee schedule (reduced rate) can be discussed if my usual rate is not affordable to you.

GOOD FAITH ESTIMATE: Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a healthcare plan, OR those who are not seeking to file a claim with their plan, a good faith estimate of what medical services will cost. You have a right to the following:

  1. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  2. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service. You can also ask your healthcare provider or any provider you choose for a Good Faith Estimate before you schedule a session.
  3. If you receive a bill that is more than $400 than your Good Faith Estimate, you may dispute the bill.
  4. Be sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your rights to a Good Faith Estimate, visit cms.gov/nosurprises
